PLEASE NOTE:
Effective June 19, 2009, the Florida State Disbursement Unit (FLSDU) will be mailing notices to inform you that future disbursements will be by direct deposit ONLY. Each petitioner will need to complete this form and return it to the Clerk’s Office Civil Court Records Support Section with a voided check or savings account deposit slip.
