Effective January 4, 2010, the following Civil forms are changing per Supreme Court Order SC08-1141:

• Form 12.928 the New Cover Sheet for Family Court Cases

These forms and coversheets are designed to be tools that help Clerk staff to better capture, determine, and report correct case type designations and Final Dispositions.
