- This application is for Pinellas County cases ONLY.
- If payment is presently directed to the Department of Revenue for distribution to you, call
1 (877) 769-0251 or visit the
Department of Revenue website for information and options.
- If your name is not listed on the void check/deposit slip you are providing, please
provide verification that the bank listed will accept transfers in your name
to the account specified.
- If there has been a name change, please mail a copy of your marriage license,
driver's license or applicable court order along with your application.
Failure to do so will result in the application being returned to you and
not processed.
Please fill out and print the
Payment Options Form and submit it with a VOIDED check (for a checking account) or a DEPOSIT SLIP (for a savings account) to:

Direct deposit will begin immediately after receipt of your completed payment option form.
Any payments received between the receipt of your payment option form and the start of the
direct deposit will be mailed to you.
Please remember, if you have a change in name or address or any other personal information which may affect your child support payments
or the child support record, you are responsible for notifying the Clerk's Office. Your changes should be put in
writing, signed and mailed with
proof of the name change to:
